Lisa Leon joined TCIL 7 years ago. Lisa started out with providing a core service to clients called information and referral. Lisa also provided office administrative support. Lisa was later promoted to Transition Manager, where she established connections with local hospitals and nursing homes. Lisa built the Home Transition Program that is used by TCIL today, to assist clients transition from an institutional setting back into the community or less restrictive environment. In recognition of her valuable contributions, Lisa was promoted again to our ADRC Program Coordinator. In her role, she collaborates with the Area 1 Agency on Aging and the California Department of Aging to create a No Wrong Door system in the County of Humboldt.
Lisa graduated with honors and a mentoring certificate from Carrington College in Sacramento, CA. Lisa holds a certificate in Medical Assisting. Before joining TCIL, Lisa worked under a doctor assisting patients in general family practice. Lisa is a mother to three daughters and serves as a Board Commissioner to the Housing Authorities of the City of Eureka and County of Humboldt. In addition to her other service positions, Lisa serves on the board as the Secretary for the Free to Fly Foundation Single Mom Fund (FFFSMF). Lisa enjoys hiking and exploring the great outdoors with her family in her free time.
