Update Q3 – Transition & Diversion Services

The Home Transition Program made significant strides in expanding awareness and access to services by reaching out to local providers throughout the community. Our goal was to deepen understanding of Tri-County Independent Living (TCIL) and the Home Transition Program, while strengthening the network of support for individuals seeking greater independence.The program is dedicated to helping individuals transition from institutional settings to more independent, community-based living environments. Through personalized, supportive services, we aim to create a pathway toward dignity, autonomy, and a higher quality of life. Additionally, the Home Transition Program offers diversion services—preventative support for individuals at risk of institutionalization. 

These services include training in essential independent living skills that empower individuals to remain in their homes and communities. Throughout the year, our team actively participated in tabling events across Humboldt County, sharing information, resources, and referrals with community members of all ages. We highlighted TCIL’s core services, which include independent living skills development, youth transitions, transition and diversion services, assistive technology, and advocacy.In furthering our transition efforts this quarter, our Transition Coordinator collaborated with the Long-Term Care Ombudsman at a local skilled nursing facility and connected with the social worker at Eureka Rehab. We provided referral materials and shared that we are currently accepting new referrals to assist more individuals in achieving independent living.

One inspiring success story involved a client at risk of institutionalization who needed help finding an In-Home Supportive Services (IHSS) provider. Through one-on-one support, the Transition Coordinator helped the client strengthen their communication and time management skills—empowering them to meet their Independent Living Plan (ILP) goals and successfully hire an IHSS worker. Through strong partnerships and compassionate support, the Home Transition Program continues to open doors for individuals seeking greater freedom, choice, and community connection.
